Acquisitions Book Request Form

One Request is allowed per form - book requests only
If you have a journal subscription, please contact the Library Director

For media requests (VHS/CD/DVD etc.) please use the media request form.
There is a limit of no more than five (5) online requests that
can be submitted in a one week period.
Fill in as much information as you have about the book.
Note that fields marked with * in red are required information.
Incomplete forms will not be processed.
