Collaboration between Residence Life and Disability Services has resulted in a new policy, effective this academic year.
Any current resident student requesting housing accommodations (for example, a single room, accessibility features or modifications, or location on a specific floor) for academic year 2013 – 2014 is required to complete an application as part of the room selection process. Housing accommodations are individually determined and based on the functional limitation(s) that are identified by a student and confirmed by a physician or otherwise qualified professional.
If you would like to request housing accommodations:
STEP 1: Complete the Student Request Form here OR print it out
STEP 2: Your healthcare provider (primary care physician, medical specialist, or therapist) must complete a form (Disability Verification Form), according to the type of disability.

Forms must be downloaded and returned to the Disability Services Office - dso@worcester.edu. Information will be reviewed by the Housing Accommodations Committee and students will be contacted regarding their requests by January 25, 2013.
