To ensure the provision of reasonable disability-related accommodations and services to qualified students, students must demonstrate eligibility and formally register with the DSO. The following steps are in place to facilitate the timely registration of students with the DSO:
1) SELF IDENTIFY AND PROVIDE APPROPRIATE DOCUMENTATION
· Students with documented disabilities who enroll at the college and who seek classroom, exam, or other accommodations must first self identify directly with the DSO and provide current and comprehensive documentation that demonstrates a substantial limitation to one or more major life activities as defined under the ADA.
· There is no automatic referral from Admissions or other WSC offices.
· IEP’s and 504 plans are not sufficient documentation to determine eligibility for services.
· Refer to the specific guidelines that are available to assist you in acquiring appropriate documentation for your disability on the website at www.worcester.edu/dso.
· Upon self identification directly with the DSO and the student’s submission of appropriate documentation, the DSO will review the documentation to determine if it qualifies the student for services and accommodations. This process may take up to two weeks to review.
2) SCHEDULE AN INTAKE APPOINTMENT
· With appropriate documentation in place, the student must schedule an intake appointment to discuss and confirm reasonable and appropriate accommodations and the process for accessing services.
· It is the student’s responsibility to follow up with the DSO to schedule an intake appointment. The intake appointment is mandatory to officially register with the DSO and subsequently receive reasonable accommodations.
