A. Required immunizations include:
1. Tetanus/Diptheria/Acellular Pertussis(Tdap):
1 dose required, if it has been 5 years or more since the last dose of Td(Tetanus/Diptheria). If less than 5 years since last Td, 1 dose of Td required within the last 10 years.
2. MMR(measles, mumps and rubella)
a. immunization with 2 doses of live vaccine given at or after 12 months of age and a second injection given at least one month apart from first immunization or(if given instead of MMR)
a. serological evidence of immunity(rubeola titre) or
a. serological evidence of immunity(mumps titre) or
a. serological evidence of immunity(rubella titre) or
3. Hepatitis B:
Vaccination with 3 doses of Hepatitis B vaccine or serological evidence of immunity to Hepatitis B.
All full-time, resident, students must meet the requirements for the meningitis immunization. Immunization with a dose of meningococcal polysaccharide vaccine within the last 5 years ( or a dose of meningococcal conjugate vaccine at any time in the past), or sign a waiver that states that the student has received and reviewed information regarding meningococcal disease/vaccination, and the student wishes to decline the immunization.
Birth in the U.S. before 1980(except Full-time/Part-time students in a health program who have patient contact), a history of chicken pox verified by a health care provider, laboratory evidence of immunity to varicella, or 2 doses of varicella vaccine (given at or after 12 months of age, and a second injection given at least 4 weeks after the first dose).
TB test required for students who have positive responses to the questions in the tuberculosis screening section of the Health Form.
B. A physical examination, by a primary care provider, within 15 months of entrance is required. The physical exam section, immunization record and the Tuberculosis Screening section of the health examination form should be completed by the primary care provider.
C. Pre-matriculation examinations are not done by Student Health Services.
D. If the Health Form is not complete, resident students at WSU, will not be allowed to move in to Residence Halls. For all other students, if the Health Form is not complete within 30 days of enrollment in classes, the student will have a Health Hold placed on their account, and will not be able to register for classes for the next semester.
E. The Health Insurance Requirement at Worcester State University is based on the Massachusetts General Law Chapter 15A, Section 18, which requires all undergraduate students registered for nine(9) or more credits and all graduate students registered for seven(7) or more credits to participate in a health insurance plan. To fulfill this requirement the student: will automatically be billed and covered by the Massachusetts State System Plan designed for state college students or if the student is already covered by another health insurance plan, the student must complete a waiver form.
