Instructions for Immunization Requirements and Compliance
All full-time undergraduate and graduate students, any international student on a student visa and all health science majors are required to upload immunization information no later than the deadline. This Health and Immunization information is required per Worcester State University policy and is in accordance with the Massachusetts College Immunization Law 105 CMR 220.600. The student is responsible for verifying immunization compliance in the student health portal.
Please review all health and wellness requirements in this document carefully. This checklist should be completed prior to the start of the semester. Failure to complete these items and satisfy all requirements on time could result in a delayed move-in process and/or registration hold.
Submitting Immunization Documentation
Immunization forms are due: August 1 (for Fall semester start) and January 1 (for Spring semester start).

- Step 1: Log on to your Medicat Student Health Portal: https://worcester.medicatconnect.com
- Step 2: Click on “HERE” for Compliance Requirements
- Step 3: Click “Upload” under Immuization Requirement
- Step 4: Under Immunization Record click “Select File” which will allow you to upload the document containing your immunization record
- Step 5: Click Upload
Submitting Physical Exam Documentation
Physical Exam forms are due: August 1 (for Fall semester start) and January 1 (for Spring semester start).
- Step 1: Log on to your Medicat Student Health Portal: https://worcester.medicatconnect.com
- Step 2: Click on “HERE” for Compliance Requirements
- Step 3: Click “Upload” Physical Exam Requirement
- Step 4: Under Provider Physical Exam Form click “Select File” which will allow you to upload the document containing your Physical Exam
- Step 5: Click Upload
Tuberculosis Screening Questionnaire
Screening questionnaire is due: August 1 (for Fall semester start) and January 1 (for Spring semester start).
- Step 1: Log on to your Medicat Student Health Portal: https://worcester.medicatconnect.com
- Step 2: Click on “HERE” for Compliance Requirements
- Step 3: Click on Forms Required for Compliance
- Step 4: Choose Tuberculosis Risk Questionnaire
- Step 5: Answer all 7 questions
- Step 6: Click Submit (blue button)
- Step 7: If the answer is YES to any of the Tuberculosis risk form questions Worcester State University requires documentation of further evaluation. If the answer to all of the Tuberculosis Risk form questions are NO, no further testing or action is required.
- Step 8: If any YES answers download and Medical Evaluation for Latent Tuberculosis Infection form on the Forms page and provide it to a health care provider for completion.
Health History Questionnaire
Screening questionnaire is due: August 1 (for Fall semester start) and January 1 (for Spring semester start).

- Step 2: Click on “HERE” for Compliance Requirements
- Step 3: Click on “Forms Required for Compliance”
- Step 4: Choose Health History
- Step 5: Answer all questions
- Step 6: Click Submit (blue button)
Other Important Immunization/Requirement Information
- Once you upload documentation and save records, Health Services will then receive your documentation for review. It can take 3-5 business days for our review.

- To view what immunizations we have on file for your, please click on Immunizations within your Medicat Portal. A red dot indicates non-compliance/something is missing.
- Compliance is met when all immunization requirements are marked with a green checkmark and “Compliant.”

- Step 3: Click Health Insurance Card “Select File” which will allow you to upload the document containing your health insurance card
- Step 5: Click Upload
