REQUIRED HEALTH FORMS
Fall 2026 INCOMING STUDENTS AND STUDENTS THAT HAVE NOT COMPLETED THE HEALTH SERVICES REQUIREMENTS:

Listed below are needed for compliance: (more details of below requirements can be found at the bottom of this page as well as for Health Insurance Waiver)
- Physical exam within 2 years as of June 2024.
- MA State Immunization Records. If you are a MA resident, your immunization records will automatically be uploaded to the student portal. It is your responsibility to check the portal to ensure the records were uploaded. For out of state students, you will need to get a copy from your physician. You may also obtain your immunizations from a personal medical portal, for example “MyChart”.
- TB (tuberculosis) Questionnaire
- Health history which the student fills out.
- Copy of current Health Insurance card (front and back).

Resident students must submit health forms by August 1. Students will not be allowed to move into the residence halls if there are outstanding requirements.
Commuter students must submit health forms by September 1.

Health Services exists to provide accessible, affordable, quality healthcare to full-time and part-time degree seeking, undergraduate Worcester State University students.
We provides diagnosis and treatment of episodic illnesses such as:
- Administration of immunizations
- Colds
- Contraceptive management and counseling
- Diagnosis and treatment of sexually transmitted infections
- Gynecologic care
- Headaches
- Health and wellness counseling
- Laboratory work
- Mononucleosis
- Nausea
- Rashes
- Sore Throat
- Urinary Tract Infections, etc.
- Vaginitis
- Vomiting
To make an appointment, call Health Services at 508-929-8875 during business hours. Students are typically able to be seen same day or next day.
If you have what you believe to be an urgent need, please call Health Services during business hours to see if we have available appointments that meet your need. We will in some cases be able to accommodate walk-ins.
There is no fee or co-pay for any care given at Health Services. Students are financially responsible for all diagnostic test fees, prescription fees and all medical services received off-campus.
