New Student Checklist

Required Forms

Submit your Health Forms Using Medicat.

  • Health History Form–Students fill out the health form, located on the student portal.
  • Tuberculosis Risk Questionnaire
    • Please answer all 7 questions.
    • If you answer YES to one or more of the questions you will need to have a TB test (downloadable form available on student portal).
    • University policy states that TB tests must be completed in the US.
  • Physical Exam–The last physical must be within 2 years of classes beginning. We do not share information with Athletics.  You will need to send a separate copy to us. Downloadable forms are available to bring to your physician if needed on the student portal.

  • Immunization Record–Please see Pre-Entrance Medical Requirements for a detailed explanation of the required immunizations. If titers have been done please submit documentation of the results.

  • Copy of Health Insurance Card (front and back)
