1. Immunizations Requirements:
The pre-entrance immunization requirements at Worcester State University are based on the Massachusetts Law 105CMR 220.000. All full-time undergraduate (12 credits) and graduate students (6 credits), all part-time OT, Nursing, and CD university students, and all students attending Worcester State University on a student visa (regardless of age) must present evidence that they are adequately immunized in order to register for classes. To fulfill this requirement, you must submit documented evidence of immunizations as listed below. Copies of immunization records will be accepted. If you cannot produce these records, you must be revaccinated, or provide serological evidence of immunity.
Please Note The Following:
1) Tetanus/Diphtheria/Pertussis:
One dose of Tdap is required, if it has been 5 years or more since the last dose of Td.
2) MMR (Measles, Mumps, and Rubella)
Immunization with 2 doses of live vaccine (given at or after 12 months of age, and a second injection given at least 4 weeks after the first dose.)
or
Measles (Rubeola):
serological evidence of immunity (rubeola titer)
Mumps:
serological evidence of immunity (mumps titer)
Rubella:
Serological evidence of immunity (rubella titer)
3) Hepatitis B:
Immunization with 3 doses of Hepatitis B Vaccine or serological evidence of immunity.
4) Meningitis:
All full-time students 21 years of age and younger, and all students (regardless of age) living in a dormitory or other congregate on-campus housing, will require immunization with meningococcal vaccine, on or after their 16th birthday or sign the enclosed waiver, stating that you have received and reviewed the information regarding meningococcal disease/vaccination, and you with to decline the immunization.
5) Varicella:
Birth in the U.S. before 1980 (except full-time/part-time students in a health program who have patient contact), a history of chickenpox disease verified by a healthcare provider, laboratory evidence of immunity to varicella, or two doses of varicella vaccine (given at or after 12 months of age, and a second injection given at least 4 weeks after the first dose) meets the requirement.
6) Tuberculosis Screening:
Tuberculosis Risk Questionnaire Form needs to be completed and returned with the Health & Immunization Form.
2. Medical History and Physical Examination Requirements:
A physical examination by a primary care provider, (within 2 years), prior to the first day of classes is required to be on file for all full-time undergraduate and graduate students and all part-time health majors. It is expected that the student will complete demographic information, health history, and health insurance information. Completed health forms from a student’s previous college are acceptable.
3. Health Insurance Requirements:
The Health Insurance Requirements at Worcester State University is based on the Massachusetts General Law Chapter 15A, Section 18, which requires all undergraduate students registered for nine (9) or more credits and all graduate students registered for seven (7) or more credits to participate in a health insurance plan. To fulfill this requirement you:
1) Will automatically be billed and covered by the Massachusetts State System Plan designed for state college/university
students or
2) If already covered by a comparable health insurance plan, you must complete a waiver form.
